Taylor Swift Sends Dakota Johnson a Warning About Colin Farrell in Chilling “Patient Zero” Video
Taylor Swift turned a friendship story into a haunting mystery in her new music video for “Patient Zero.”
The Grammy winner premiered the cinematic video during the 2026 MTV Video Music Awards on Sept. 27, bringing together Swift, Dakota Johnson and Colin Farrell for a story about love, betrayal and a warning that comes from beyond the grave.
In the video, Johnson plays a woman who finds herself in a troubled relationship with Farrell’s character. Swift appears as his former lover — but there is a major twist: her character is already dead.
A Ghost With a Warning
Throughout the video, Swift’s character watches from the shadows as Johnson's relationship begins to unravel.
The story gradually reveals that Farrell’s character was once married to Swift’s character, who died under mysterious circumstances. Johnson's character eventually visits her grave, unaware that the woman buried there has a connection to the man she is dating.
Swift's ghost becomes a silent witness to the same warning signs she experienced herself, ultimately trying to guide Johnson away from the fate she suffered.
The result is part gothic ghost story, part psychological thriller and part relationship cautionary tale.
The Story Behind “Patient Zero”
Swift has explained that the song was inspired by a real-life conversation involving a friend.
She recalled being with a group of friends when one woman received a message from the new girlfriend of an ex, asking whether she had experienced similar behavior from him. The women began comparing stories and realized several of them had gone through similar experiences with the same man.
That idea became the foundation for “Patient Zero” — essentially a conversation between women who recognize the same patterns in a relationship and try to warn the person who comes next.
Dakota Johnson and Colin Farrell Join the Story
Johnson and Farrell bring the fictional relationship to life, while Swift takes on a role that is much more involved than a typical music-video cameo.
Johnson, who has been friends with Swift for years, said the two decided to work together because of their longstanding friendship.
“I was hanging out with a bunch of friends,” Swift previously explained of the inspiration behind the song, describing how the idea of women comparing notes about a shared ex sparked the concept.
The video also features Cara Delevingne, who appears as another woman connected to Farrell's character.
A Cinematic Collaboration
Swift directed the video, with Oscar-winning cinematographer Emmanuel Lubezki, whose credits include The Revenant, Birdman and Gravity.
Swift has said she wanted the visual to explore several genres she loves, including gothic ghost stories, true crime and psychological thrillers.
The elaborate house featured throughout the video also carries symbolic meaning. Swift explained that it represents the coldness of a relationship that has gradually faded.
Part of The Life of a Showgirl: The Encore
“Patient Zero” is one of four new songs on The Life of a Showgirl: The Encore, the expanded edition of Swift’s album released Sept. 25.
The additional tracks include “Cleveland!,” “Pink Clouding” and “Babylon.” Swift reunited with longtime collaborators Max Martin and Shellback to create the new material.
The video premiere also came during a major night for Swift at the VMAs. She received the inaugural Artist Director Honors, recognizing her work behind the camera and contributions to music-video storytelling.
A Warning Wrapped in a Ghost Story
By the end of “Patient Zero,” the message becomes clear: sometimes the person who understands a dangerous relationship best is the one who already survived it.
Only in Swift's version, the woman doing the warning is no longer alive to tell the story herself.
